A report into the failed Toy Show Musical has found there are differing views about whether the RTE board approved production in the first place.
Chair of the RTE Board, Siun Ni Raghallaigh, has apologised to the public and to the staff of RTE.
It shows the production lost a total of €2.272 million.
In a statement following the publication, Chair of the RTE board Siún Ní Raghallaigh, said board approval was required for the musical but formal approval was neither sought nor provided for.
Miss Ni Raghallaigh says the board was not kept appropriately informed about the development of the musical and says external advice was ignored and information was withheld from the board.
She said the relationship between the board and executive has been redefined, with risk assessment now central in all decision making and a formal approval process established for projects with significant expenditure.
She apologised to the public and staff at RTE and said the board is fully focused on restoring confidence in the organisation
