The collision between a lorry, tractor and car happened on the N5 in Frenchpark at 6:15pm yesterday.
The lorry driver was seriously injured and airlifted to University Hospital Galway for treatment.
Gardaí are appealing to anyone who was travelling in the area at the time to contact them.
